You will be seen by a Physical Therapist for an initial evaluation. Your medical history will be discussed, and your Physical therapist will perform palpation, range of motion, posture assessment, special tests, neurological screening, and more.
Please wear loose fitting clothing in order to expose the body part that will be evaluated by the physical therapist.
Yes, your physical therapist is licensed. Dr. Jean Dornevil’s PT license number in Florida is PT30933
Medicare does require a physician’s referral to pay for physical therapy services.
Private Insurance: some plans do require a physician’s referral, and some do not. It is always best to call your insurance company to verify.
Self-pay: Florida currently allows direct access to outpatient physical therapy services for up to 30 calendar days without a physician referral. If you need to continue your care after this timeframe then you can get a referral from a medical professional such as: MD, DO, Physician Assistant, Nurse Practitioner, Dentist, Podiatrist, or Chiropractor.
Therapy visits typically last between 30 – 60 minutes.
Make sure you bring your physical therapy prescription that your doctor gave you. You will also need to bring your insurance card(s). If you owe a co-pay, deductible or co-insurance please bring a form of payment with you as we collect each date of service. We accept Visa, Mastercard, Discover, American Express, Check and Cash.
