No provider-patient relationship
Viewing this website, submitting a form, or exchanging email or phone messages with us does not by itself create a provider-patient relationship. That relationship begins only when you have been formally accepted as a patient, have completed the required intake and consent documentation, and treatment has begun.
Communication is not secure
Website forms, ordinary email and text messages are not secure channels. Please do not send detailed medical information, insurance identifiers or other sensitive data through them. Use the telephone instead, or wait until we can provide a secure method.
Appointments and cancellations
Appointment scheduling, cancellation requirements, no-show policies and fees are set out in the documents you receive as part of intake. Because we are a mobile practice that travels to you, advance notice of cancellation is particularly important, and our cancellation policy will be provided to you in writing before treatment begins.
Acceptable use
You agree not to use this site to do anything unlawful, to attempt to gain unauthorized access to it, to interfere with its operation, to scrape or harvest data from it by automated means, or to submit false information or content that infringes the rights of others.
Intellectual property
The design, text, graphics, logos and photographs on this site are owned by Aktive Motion Physical Therapy or used with permission, and are protected by copyright and trademark law.
